Veteran of the Month Guidelines and Nomination Forms
The Veteran of the Month award recognizes Nevada veterans who contribute their time and energy to support the veteran and military communities.
"Men and women across Nevada who have made a significant contribution will have achieved excellence by going above and beyond the call of duty specifically with respect to volunteer work in support of veterans and the military, we're going to recognize their service," stated Governor Sandoval in his presentation. "It will be an opportunity for all of us to shine a bright light on the work veterans continue to do for our nation."
The program is managed by the NOVS and we continue to ask for your help in nominating possible candidates so we may recognize veterans who contribute time and effort for the betterment of our state and the veteran community. Submissions will be reviewed and chosen by the members of the Veterans Service Commission (VSC) at its quarterly meetings.
There are two avenues to submit nominations; Paper or Online
Paper:

2. Download and complete Nomination Form - VOM Nomination Form OR
Fill out and print. - Online PDF Nomination Form
3. Download and complete Contribution Sheet - VOM Nomination Contribution Sheet
4. Obtain a copy of DD Form 214 All nomination forms must include the Military Service Record (DD Form 214).
5. Mail to: Nomination forms can be submitted by:
